More clarifications -
The first 9 clues each have a one-word answer.
These words fill in the 9 spaces at the bottom (1-1 through 3-3).
These words, along with the LA,MD,DE clue are hints to the event that is the solution to the puzzle.

I am sorry if this is redundant, but once we fill in the blanks, are we completely done with the clues labeled 1-1 thru 3-3?

There have been a few additional questions regarding how to use the clues.
Basically, the first 9 clues lead you to the 9 words that go into the spaces. I can't really tell you how to use these words once they are there because that is part of the puzzle.
Once you have obtained these words, you no longer need the first 9 clues - those are only to obtain the words. You are now working with just the 9 words as shown in the bottom three lines. And the LA,MD,DE clue.
These 9 words and the LA/MD/DE clue lead you to one event. This event has a specific name, like "the Jamestown Flood ", or "World War II", or "the Salem Witch Hunts" (these are not clues to this puzzle, just examples of events with names).
Finally, the bottom line - 3-1, 3-2 and 3-3 are the names of actual people & a place.
I will add a hint in a few days if I don't have any correct solutions.
Feel free to send me your list of 9 words for verification. I can't give you the answer, but I can tell you if they are right or wrong so you know if you are searching with the correct information.

I will release my first hint to Bad Omens tomorrow, Thursday 12/10 at 2pm EST if I have not received four correct answers by that time.
Many of you have sent in some really great stories as possible solutions, and I'm amazed that so many interesting bits of history have been found using my set of clues. Although so far only one of these was right (congrats #1!), I'm taking notes, so one or two of these stories might show up in a future hunt!
